Autism Stem Cell Treatment Research: Who is the Real Quack? Gorski (ORAC) or the FDA?

In any commentary in which I dare question the self appointed protector of all things scientific, the ORACle known as Dr. David H. Gorski,  I have to begin with the statement that I recognize the public health importance of vaccines and that both of my sons, and I for that matter, have received all vaccines recommended by our local public health authorities in New Brunswick.  This statement is necessary because Dr. Gorski almost invariably alleges that anyone who questions the wisdom he brings down from the mountain top for us, the ignorant unwashed, is really just a closet anti-vaxxer.  I don't think vaccines are perfect though and it is my understanding that neurological damage has been caused by some vaccines in some individuals. I am aware also that Dr. Bernadine Healey had recommended further study of possible vaccine autism connections before her death ... and that some had vilified her for doing so.

Yesterday I commented on the fact that the FDA had approved a study to determine whether umbilical cord blood derived stem cells might be effective tools in treating autism. The article I quoted from contains some professional quotes indicating that the study is well designed and also contained a number of quotes expressly indicating that it is a very PRELIMINARY study.  I was happy to see research aimed at establishing or assessing possible treatments for autism disorders.  I was also happy that the study was, as reported in the article, approved by the US FDA.  To my thinking, as a humble autism dad,  FDA approval confirmed that the study would be conducted by appropriate guidelines, professionally and ethically. My commentary attracted a comment from MJ, author of the Autism Jabberwocky blog:

"Did you notice that the self-appointed high priest of science wrote (at least) two pieces on stem cells and autism? In the first he calls it quackery because it isn't evidence based and in the second says that studying the issue at all is unethical. He really seems to want to have it both ways. No treatments without proper research, which is reasonable, but no research because there is no evidence that it would work...."

I think MJ has made an excellent point.  How are we supposed to determine what is evidence based, effective treatment for autism or any other disorder, if no research is done to determine effectiveness?

Gorski`s objection to the ethics of a preliminary study of stem cells in treating autism disorders is set out in Is a trial of stem cell therapy in autism scientifically and ethically justified? :

``Think of it this way: Do you think that the evidence implicating a hyperactive immune system is strong enough to justify treating autistic children with prednisone? Cyclosporine? Other immunosuppressive drugs? If not, then why would anyone advocate using autologous stem cells, which appear to be immunosuppressive? Why on earth would an institute like the Sutter Neuroscience Institute carry out such a trial based on low prior probability? What sort of preclinical evidence did they have to justify this trial? The scientists in the article who say that the likelihood of a positive result from this trial is low are, if anything, too optimistic. The likelihood of a positive result is almost homeopathically low. When it comes to clinical equipoise, this trial looks to me as though it’s all risk with too little prospect of benefit to be justifiable without a lot more clinical evidence.``

While he is notorious for splashing for attention by flapping his wings and quacking out cheap insults at those who disagree with him, I have no doubt that surgical oncologist Dr. David H. Gorski (ORAC) is much better placed than I am to assess the ethical basis for conducting this preliminary study.   He questions the internal review board (IRB), apparently unknown to him at the time, in  a reasonably courteous, serious  manner.  What is not really clear from Gorski's comments is whether he would ever view any autism treatment study as justifying  supervision by an IRB or approval by the FDA.  In the comment linked above he did state with reference to pharmaceutical companies seeking drug approval:

"Similarly, any private entity (such as a pharmaceutical company) seeking FDA approval for its drug or device have to register with the FDA and abide by the Common Rule, whose most important set of rules mandate IRB approval and monitoring of the research. Some states also mandate that all human subjects research carried out within their borders, regardless of funding source, must abide by the Common Rule."

Since there is an IRB in place for this preliminary study, and since the FDA has given its approval for it to proceed, I assume Dr. Gorski (ORAC) will, if he has not already done so, acknowledge that this study is being conducted in accordance with recognized public health authority backing. As a mere layperson I assume that the US FDA has conducted the usual inquiries and that they are best placed, even better placed than surgical oncologist and autism expert Dr. Gorski, to determine the ethical appropriateness of the study. 

I assume that Dr. Gorski who feels FDA approval is a comforting requirement for  approval of new pharmaceutical company drugs or devices is also comforted by the FDA approval of the stem cell preliminary study. Surely he does not now consider the FDA to be governed by the quacks he so detests?

Regardless, if Gorski, the FDA or any other health professional or authority is simply going to ridicule and dismiss attempts to conduct any research into possible autism treatments and cures they will simply lose credibility with many autism parents when they try to convince them that they should stick with evidence based treatments for their autistic children.  That is not quackery. That is reality.

Autism Dangers Result in Lives Lost to Autism

In Jabberwocky of the Day : Don't Speak Ill of Autism  MJ  has provided a clear and concise picture of the dangers that often accompany autism disorders.  He did so in response to a comment by Kevin Leitch, aka Kev,  at LBRB who has criticized Ginger Taylor for launching a new site called Lives Lost to Autism.  Mr. Leitch has a long history of romanticizing "autism" and criticizing anyone who views autism disorders as   ...  disorders.  I agree with MJ's critique of Kev's views as "just plain silly" in so far as they complain of unfairness to "autism" a mental disorder, not an entity or person. In describing the dangers that accompany autism disorders MJ states:

"Autism is not a good thing and has in fact directly caused many deaths.


Let me put it this way, if someone dies as a direct result of being shot would you try to claim that it was the bullet and not the person pulling the trigger that is to blame? In a similar fashion, autism robs people of abilities or facilities that they would normally be expected to have and it is this lack that can directly lead to death.


Autism can prevent children from understanding their environment and that lack of understanding can very easily be fatal. Autism can easily prevent people from being able to understand what effect their actions have on others and thus lead to their deaths. This likely happened in the case of Sky Walker - do you think he killed his mother or was it his autism that lead to her death?


I have seen many attempts to whitewash the spectrum but this one takes the cake".

I have added the bold emphasis in the above quote.  I have commented several times on this site of the time when my son Conor  left our home without me realizing he had gone. I had received a business call and was on the phone when he left. I thought I would hear him if he went out but I was wrong. When the call ended and I looked for Conor he was nowhere to be found.  I ran around our house and yard before calling 911 and being directed to a nearby convenience store. He had crossed a busy parking lot and street oblivious to the dangers of automobile traffic before a good citizen stopped and took him to the convenience store and safety.  As the adult responsible for him that day I felt intense fear and guilt that I will never, ever forget. Conor, because of his autistic disorder, does not understand and appreciate the many dangers of our everyday environment.  Autism is very, very dangerous for many autistic children.

In starting the Lives Lost to Autism site Ginger Taylor is performing a valuable service to autistic children and their families reminding us of the very dangerous realities that often accompany autism disorders.Thank you Ms Taylor. And thank you MJ for your clear statement about the realities of autism dangers. Kev, well, you have a good day Kev, And may you someday wake up and quit romanticizing autism disorders.

Autism Jabberwocky Documents Neurodiversity Hypocrisy

As I have noted before,  Autism Jabberwocky may be the best autism blog on the internet. It is certainly one that I read regularly. It is always well written and genuinely fair and balanced. None of which prevents blog author MJ from taking clear positions on issues.

In two recent posts, Left Brain Right Brain : The Bullies On Bullying and Neurodiversity's Hypocrisy, Autism Jabberwocky hits Neurodiversity ideologues on the hypocrisy that permeates their writings. . I recommend both posts, and the Autism Jabberwocky blog generally, for readers seeking an informed balanced and no nonsense perspective on autism issues.

Autism Jabberwocky Questions Michelle Dawson's Understanding of Evidence Based Medicine Concept

Autism Jabberwocky one of the best, and best written, autism blogs on the internet features an excellent commentary  which highlights an apparent lack of understanding by anti-ABA ideologue Michelle Dawson of the concept of evidence based medicine.  As explained by MJ at Autism Jabberwocky in Michelle Dawson Writes A Letter.
:
"Evidence-based medicine is the idea that all medical decisions should be based on the best scientific evidence that is available.  The concept is really very straightforward. You take the results that research has provided, rank them according to the quality of the information, and use that ranked evidence to decide what the best course of treatment is."

Making medical decisions as to appropriate treatment based on the best available evidence seems simple enough to grasp.  It is an eminently practical concept.  People can not just wait  decades  for perfect research  studies to be conducted under perfect conditions (which may never occur).  They have to make  treatment decisions based on the best evidence of safe and effective treatment  available at the time they are confronted with a medical condition requiring treatment.

It is one thing for Michelle Dawson, researcher, to hold out for an ideal and perfect study that may never happen.  It is another thing altogether for parents trying to help their autistic children overcome their disorders and live the fullest, happiest life they can. They have to make decisions to help their children based on the state of knowledge at the time.

MJ summarizes concisely and accurately the status of ABA as an evidence based, effective treatment for autism disorders and the curious nature of Michelle Dawson's apparently irrational opposition to ABA: 

"One of the few treatments for autism that does have a solid evidence base is ABA (Applied Behavior Analysis).   While it is not guaranteed to work for everyone, the available evidence shows that it can be an effective tool to help teach children with autism and is almost universally recommended.

That is, with the exception of the universe of Michelle Dawson.  As I have pointed out before, Ms Dawson has a real problem with ABA. She seems to have an almost irrational obsession with proving that ABA is somehow unethical or immoral to use on children with autism. She would tell you that she has ethical concerns and that there is very little evidence that ABA works.  However, Ms Dawson is almost universally alone in her opinion."


Michelle Dawson is not completely alone though.  Her anti-ABA views are shared by her collaborative colleague Professor Morton Ann Gernsbacher.  Gernsbacher's anti-ABA views have been scathingly reviewed by Professor Edward K.  Morris of the University of  Kansas who commented on the harm caused by Gernsbacher's misrepresentation of ABA:  A Case Study in the Misrepresentation of Applied Behavior Analysis in Autism: The Gernsbacher Lectures.

Michelle Dawson has spent her post Canada Post career  trying to tarnish the public perception of ABA as an effective, beneficial autism treatment. To my knowledge she has never explained what autism treatments, if any,  do meet with her approval.

Fortunately  the serious and credible reviews of autism interventions that have been done over the past decade and a half do not appear to give her views, or those of her colleague Morton Ann Gernsbacher,   any weight or mention.

Autism Jabberwocky Puzzled by Michelle Dawson's Obsession With Trash Talking ABA

Autism Jabberwocky (MJ) expresses his puzzlement with Michelle Dawson's opposition to ABA as an intervention for autistic children in Michelle Dawson. MJ notes the overwhelming evidence against Ms Dawson's anti-ABA position, the lack of a clear basis for her ethical concerns, and the lack of any reported life experience with ABA that would explain her views and actions in opposing ABA.

MJ also notes the fact that Michelle Dawson "has a bad habit of discouraging parents from using ABA to help their children". As anyone who reads this blog knows I share MJ's concerns about Michelle Dawson's ABA (anti0ABA) views, particularly when parents of newly diagnosed autistic children are discouraged from seeking ABA help for their children.
